The New Paradigm: High-Quality Weight Loss on GLP-1s

This 52-page document introduces a paradigm shift in obesity treatment. Instead of focusing on losing weight, the authors introduce “High-Quality Weight Loss”—the notion that we need to be concerned not just about weight loss, but also what is lost. 

The authors state: “The future of obesity treatment lies not in greater weight loss per se, but in achieving better weight loss—defined as metabolically favorable, functionally responsible, and clinically sustainable.”

We should focus on losing fat while trying to maintain as much lean muscle as possible. Yes, this is obvious, but it becomes more important with the advent of GLP-1 drugs as significant weight loss has become much more achievable.

  1. Focus on more protein (aiming for 0.7 grams of protein per pound of body weight daily) and quality of protein. The authors state, “High-quality sources such as dairy proteins, eggs, fish, lean meats, soy, or targeted oral supplements become particularly useful when total intake is constrained”

  2. Resistance exercise must be prioritized. “If high-quality weight loss is to remain more than a conceptual aspiration, then resistance training has to be treated as a central therapeutic tool rather than as an optional lifestyle accessory.” The authors go on, “Resistance training improves not only the amount of tissue preserved, but also the usefulness of that tissue.”

  3. Build the reserve. Weight loss induced by a GLP-1 will impact an active, strong, 35-year-old differently than a sedentary 65-year-old. The 65-year-old might be starting the GLP-1 journey with very low muscle reserve. Having less muscle to begin with means that they have less to lose. The authors argue that intense strength training is even more important for those individuals who are starting GLP-1 therapy with low reserves.
